Almost, but not quite. A flat treadmill is a little easier than the open road. So the pace on the screen can look better than your real outdoor pace. The gap is small, but it is real.
The main reason is air. Outdoors, you push through the air in front of you. That push costs energy. On a treadmill, the air stays still while you run in place. So the same belt speed costs a bit less than the same pace outside.
Jones and Doust measured this in 1996. They had trained runners run both outdoors and on a treadmill at many speeds. They found that on a flat belt, the oxygen cost was lower than the road at faster paces (Jones and Doust, 1996). Less oxygen cost means less effort. That is why a flat treadmill feels easier.
The gap depends on speed. At slow, easy paces, a flat belt was about the same as the road. At faster paces, the flat belt was clearly easier. So the faster you run, the more the screen flatters you.
Why does the treadmill feel different?
The treadmill removes most of the things that make outdoor running hard. There are four big ones. Each one changes how the run feels.
First, no air resistance. Outdoors you fight the air, even on a calm day. The treadmill takes that away. This is the largest reason a flat belt is easier.
Second, the belt helps a little. A common belief is that the moving belt pulls your leg back under you. Studies show belt speed is not perfectly steady. It slows a touch at footstrike and speeds up at toe-off (Van Hooren et al., 2020). This can shift how you run by a small amount.
Third, the world does not move. Outdoors, you see the ground pass by. On a treadmill, the scene is fixed. Your brain can read this as harder or boring, even when the body is working less.
Fourth, the surface and weather never change. No wind, no hills, no heat, no cold, no turns. The effort is steady the whole time. That steadiness is nice, but it is not the same as real roads.
Even with these changes, the running motion itself is close. A large review found that most movement measures are largely comparable between a treadmill and the ground (Van Hooren et al., 2020). The biggest gaps are at the foot and ankle, not the whole stride.

What incline equals outdoor running?
A 1 percent incline. That is the number to remember. Jones and Doust found that a 1 percent grade most closely matched the energy cost of outdoor running at faster paces (Jones and Doust, 1996).

But the right incline changes with speed. In the study, runners went from about 9:08 per mile up to about 5:21 per mile. The flat belt matched the road only at the two slowest speeds. Once they sped up past about 7:10 per mile, the flat belt was too easy, and 1 percent matched the road instead.
So here is the simple rule. For easy runs slower than about 8:00 per mile, a flat belt is close enough. For moderate to fast runs, set 1 percent. At very fast paces near 5:30 per mile, the road cost fell between 1 and 2 percent, so a touch more incline can help.
One warning. The incline on a gym treadmill is not always exact. Some read high, some read low. So treat 1 percent as a smart default, not a perfect lab setting. If your treadmill cannot set an incline, just know your screen pace is a little soft.
How much does air resistance really cost?
More than most runners think, and it grows fast with speed. Air resistance is the single biggest reason a flat treadmill is easier than the road.
Pugh studied this decades ago. He found that pushing through the air can take about 7.5 percent of your total energy at middle distance race speed. At sprint speed, it can reach 13 percent (Pugh, 1971). That is a large slice of the work, and the treadmill removes it.
The key word is speed. Air resistance rises sharply as you run faster. At an easy jog, the air cost is tiny. At 10K race pace, it is real. At a hard sprint, it is huge. This is why the treadmill gap is small for easy runs and bigger for fast runs.
This also explains the table near the top. At 9:00 per mile, the flat belt is about the same as the road. At 6:00 per mile, the flat belt can flatter you by roughly 12 seconds per mile. Same belt, but a much bigger air gap, because you are moving faster.

How do you convert treadmill pace to outdoor pace?
Use a simple plan based on your speed and your incline. You do not need fancy math. You need to know two things: how fast you are going, and whether the belt is flat.
If you run easy and the belt is flat, your outdoor pace is about the same. No real change is needed. Easy is easy in both places.
If you run moderate or fast on a flat belt, add a few seconds per mile to guess your true outdoor pace. A rough guide is about 5 to 15 seconds per mile, with the bigger number for faster running. This matches the air cost that Pugh measured.
If you set the belt to 1 percent, you can read the screen pace as your outdoor pace. The incline pays back the missing air cost. That is the cleanest fix, and it is why so many coaches suggest it.
